This is very rare -
If the water quality is not livable, then the axolotl will go through metamorphisis. Bear in mind they are neotenous, meaning they are 'stuck' in their larval form. It is so very rare that an axolotl will go through metamorphisis, and they only live a max of eight years after that. Most die inthe process. Again, thankfully, metamorphisis is very rare for them. Some pet owners have come to believe that if they slowly lower the water level it will force their axie into metamorphisis. This will kill the poor creatures.
Frequently tiger salamander larva are mistaken for axolotls. Sometimes people accidently take home a tiger salamander larva (let's call them TSL's), thinking they are taking home an axolotl. Then it goes through metamorphisis, which happens to all TSL's, and they freak out and get excited in thinking they are soooo lucky, until someone brings to their attention that it is orange with black stripes. That, my friends, is a tiger salamander. Axolotls that go through metamorphisis keep their coloration, whereas tiger salamanders start out looking like a wild type (it's a color morph) axolotl. They later get orange with black stripes.
